On or before August 1, 2011, the application fee is $10.00.
After August 1, the application fee is $25.00
Applications and fees are due no later than August 15, 2011.

Fee for One-Time Use of Membership List

Persons wishing to purchase the Leadership Oklahoma City membership list must submit a completed form, available above, and a fee. Amount of the fee is shown in drop-down menu.

The fee may be submitted online line via Google Checkout, below, or by check or credit card information submitted directly to Leadership Oklahoma City.
